You will now be presented with the form below
You need to complete this form as shown above and an 'Account Activation' email will be sent to you at the address you entered. If you don't get the email within a few minutes please check your spam/junk email folder for it. If you still don't find the email please contact me at the address in the image below (sorry it is an image but it stops the spam bots from getting my email address) and I will check your account settings and activate you account for you.
