You need to write to the above address enclosing a copy of the Death certificate of the rank (unless they died in service) and a copy of your birth certificate proving you are the next of kin. If you are the widow you must send a copy of your wedding certificate to prove you were the spouse at the time of death. Unless you are the widow there is a £30 charge for the service.
